Cadet Forms
Anyone who participates in ROTC is required to complete and submit the forms below before they may be involved in training exercises and other activities.
Please note: If you have trouble opening any of these forms, send an email specifying which form to rotc@sbu.edu. Someone will be in touch with you to provide assistance.
- Briefing on Government Sponsored Benefits for ROTC Cadets
This cadet enrollment form determines a student's eligibility to be enrolled in ROTC. - Authorization/Declination for Access to Student Records
This form allows the ROTC Cadre to request grades and transcripts from the registrar's office. - Cadet Application & Enrollment Form
This form is reviewed by the Cadre and determines a student's eligibility to be enrolled in ROTC. - Medical Fitness Statement
This form is signed by a physician stating that a student has no medical conditions prohibiting participation in ROTC. (ROTC activities are no more strenuous than those of a regular college physical education program.) - Dental Records Form
This requests a student's dental information, which is required so that students may ride or fly in government vehicles. - Digital Signature
